names.txt files issue

I created a new names.txt for a fictional world along with a first_names.txt and world_default.xml file.
Everything appears to work as I expect except for one thing.
Some of my players last names come from the names.txt file but most of the players name my last name.
Any idea why this would be the case?

Because names.txt is for last names. first_names.txt is for first names.

Maybe I wasn't completely clear.

I have a names.txt file and a first_names.txt file.
First names are coming from the first_names file with no issues.
A few of the last names appear to be coming from the names.txt file as well but most of the players all the the last name 'Napoleon' which is mine (as well my my GM name) name.
Why would it be picking up my name?

Try posting the file here that's giving you trouble so some of us can get a look at it.
