I'll add to this as I find more:

Pitcher Management Error: In Spring Training, a starter was pulled after 3 innings (makes sense, he had a bad performance). Closer (with 9/20 endurance) is then inserted and pitches TWELVE innings in extra inning game and takes loss. No other pitchers were used in the game by that team.

This was TT # 1448, and it has been fixed in patch 1.
