Online League Reports Issue

When I input my server information into the online league settings than try to create reports using the MySQL data base it keeps giving me an error. Also it happens when I try to save the game. I attached the screen shot to the message so you can see. I have tried it a few different times but it won't let me upload the reports and save the game.
