This happened frequently in PT19 too.

It's just a bug with the image. The actual card can be found in your card list as normal. The frustrating part is trying to figure out which one card is new out of hundreds in your collection.

It happens when your system and the server lose their connection, usually due to an internet blip or timeout.

The cards are never lost, but as 2002halos says, it can sometimes take Sherlock Holmes level detection work to figure out which card in your reserve/inactive list was the blank one.
