Lg > Settings > Rules > Reliever Minimum Batters Faced

It's on by default when you create a modern league (MLB or fictional), but not a historical one before some year (2020?). So it might be on or off depending on how you created your league or changed the setting.

And I think the rule says a reliever who gets the last out of an inning does not have to come back into the game even if he faced fewer than 3 batters.

You can turn it off. I do because I like those sweaty late inning moments when every at bat is crucial. I miss that in MLB.

It's a dumb rule. I turn it off in every sim, along with the limits on pickoff throws, and the ghost runners in extra innings. On the other hand, I am okay with limits on iF shifts. Let's play the game the way it was meant to be played, without the obvious gimmicks.

That said, while it's irrelevant to OOTP, I was 100% wrong in my curmudgeonly opposition to the pitch clock. It has been seamless. Everyone likes the faster games. It has not interfered with play at all. In an amazing way, it gives MLB credibility in trying to improve its product by listening to people.

Wasn't aware this had been added, where in the game is the setting for it?

The pickoff rule isn't in the game.
