Pulled a "blank" card? See here.

Sometimes you will see that a blank card comes up during pack opening. This is very rare, but it happens when there is a short connection problem between the server and the client, we cannot prevent this, it's just the way the internet is.

This does NOT mean that you lost the card. It still ends up on your roster or your inactive cards list (see here: http://www.ootpdevelopments.com/boar...d.php?t=300134) although it will take a bit of Sherlock Holmes work to find out which card was actually pulled. You'll notice it though in your card list if it was a good one

Is this what is considered a blank card? I can't find what this one may have been...I feel I am missing a silver?

And another one...must be why I haven't gotten much above bronze in the last 20 packs or so.

Yes, those are blank cards. It's just the image that did not get properly downloaded for the pack opening scene, the card it either on your reserve roster or the complete card list.

Is this a thing? I purchased 3 regular packs and upon opening the first it hung out on the stadium background screen and no card pack with the opening animation screen. It sat there for about 2 minutes and then just jumped back automatically to the home screen. It still showed three unopened packs and so I tried to open the first pack again. This time it opened regularly, but the button for open next pack only showed one other unopened packs. So, I'm not sure where my third pack went....I was able to open 2 packs, but the third one disappeared.

When this has happened to me, I've been able to click on the card and the hover shows the name and base ratings for the player (same as if you clicked on any of the other cards). So, you should be able to easily find out who it is.
