It's a Pittsburgh Penguins font. Yearbook is similar.
EDIT: As for how to use it, it would depend on what you want to use it for. Like are you wanting to make your own jerseys using eriqjaffe's uniform maker or...?

Ah okay. As to answer your question, I was wanting to use it for the uni maker so, yeah I was wanting to use it for the maker.

Okay, download the font of your choice, install the uni maker if you haven't already, and press the button that Deja Bru points to in this post. If you have trouble installing or using the uni maker, just look at Deja Bru's previous posts in that thread. He made a great step by step tutorial.
